Application form active participants 2010

IMPORTANT - recently in 2010 we occasionally had problems receiving the form emails. Therefore to be sure your application has reached us, we will confirm each application by 'normal' email within one or two days, apart from the automatic copy you should receive after submitting. When we do NOT confirm by 'normal' email, contact us on info@tihms.com to be sure. Also, when you do not receive an automatic copy of your application, this is reason to be alert on this.
In case we did not receive your application and you did receive your copy, best thing to do is to send through to us this copy of your application.

Completing the application form

Please note the following practical considerations when completing the application form:

The form opens in a new window, which you can size screen-wide. This way you 'escape' the frame pane which is smaller, and on low-resolution screens this is an advantage. To go back to the site after completion, just close the window, or choose the window you came from.

(On some older browsers a new window may not 'show'. For that reason we have implemented an option to open the application form in this same window. To use this option, please scroll down to the bottom of this page.)

After the form page is loaded, you may go 'off line' (turn off your internet connection), and take your time to complete it. Pressing the [send] button brings you online again.

After you press 'send', a 'thank you' page will appear, confirming that the form has been sent. Also, a copy of the E-mail will be sent automatically to your own E-mail address.

Copy and paste text blocks

Some fields require several lines of information, e.g. the biography information. You need not retype all this text. When you have prepared this text in (for instance) a word-processor, select the text in the word processor and copy it to the computer clipboard with [Control]+[C]. Then go to your browser and to the form. Set the cursor in the appropriate text-field, and paste the text from the clipboard into the field with [Control]+[V].
(On Macintosh computers read [Command]+[C] and [Command]+[V].)

The same method (copy and paste) can be used with the listing of the recital program. However, this type of information is often formatted, and set in columns to make it look attractive. When copying this information into the application form, it is most convenient for us when it is listed in the most simple way, without tabs, without indents and without lines of multiple spaces. An example:

It is advisable to make such a list in this manner beforehand, so that you can copy and paste it to the application form in one block. There are some other fields too that may require some preparation in advance which you will see if you take a look at the questions asked.

Required fields

The form requires several fields to be filled in. These fields are marked with an asterisk (*). When you forget a required field, using [send] gives you a warning that required fields are missing, starting from above. The cursor will be set in that missing field. When all fields are completed, using [send] will bring you to the 'thank you' confirmation page.

Send lock

On the form you will see a button [Send is disabled] just above the [send] button. You will have to change this button to [Send is now enabled]. We implemented this because in some browsers (Explorer also) the Return-key or Enter-key has the effect that the form is submitted (or in other words, sent). Often this is not intended, and this system insures that the form cannot be sent by accident before it is fully completed.

Your browser and javascript

We use javascript in the form to validate it and to make the automatic calculations. Javascript is a kind of programming language that browsers use. Please make sure javascript is enabled in your browser. If not, the form provides a warning that it isn't. Javascript is enabled by default, but if it's off, you can turn it on in the 'preferences' pane of your browser.

See here if javascript is DISabled. Then a message occurs in the box below. If the box is blank, javascript is enabled so you can use the form.

Your own copy of the form

• You will need two copies of the form in print. One copy to sign and send to TIHMS (the address is in the form), and one copy for yourself. You may make these print copies just before pressing [send] or after. In the latter case, after pressing [send], the confirmation page appears, but the filled in form is not gone. Retrieve it, in other words, get it back, by using the [back] button of your browser, and then print it. (After printing you may close the window.)

• If you entered a valid E-mail address in the form (beware of type errors..), this E-mail address will be used to send you an E-mail copy of the form you submitted. This E-mail copy is sent to you within a minute after submitting (sending) the form. It contains the same information as the form itself. Therefore this E-mail copy may also be used (fully completed and signed) to send to TIHMS by mail.
